Opisthorchiasis

Opisthorchiasis is a disease from the group of trematodes that affects the liver and pancreas.

The reasons

A person becomes infected by eating slightly dried, lightly salted, undercooked or raw (stroganina) fish. In the human intestine, the larvae are released from the membranes, penetrate through the common bile and pancreatic ducts into the liver, gallbladder and pancreas. After a month, the parasites reach sexual maturity and begin to secrete eggs. The fluke lives in the human body for many years.

Symptoms

In the early stage of the disease – fever, urticaria, aching muscles and joints, later – pain in the right hypochondrium, in the stomach; often enlargement of the liver and gallbladder.

Diagnosis

Diagnosis of opisthorchiasis in the early phase is difficult, because the eggs of parasites in the bile and feces are found only after 4-6 weeks from the moment of infection. Therefore, the diagnosis of acute opisthorchiasis is based on epidemiological history (staying in the outbreak, eating raw, dried, lightly salted fish there) and clinical manifestations of an acute allergic disease with fever, skin rashes, myalgia, arthralgia, catarrhal other phenomena against the background of leukocytosis and eosinophilia.

The most effective methods of coprooovoscopic diagnosis are the method of ether-formalin deposition of eggs, the method of ether-acetic deposition and a thick smear according to Kato.

Treatment

Treatment of patients with opisthorchiasis, both in the acute and chronic phases, should be comprehensive, observing the principle of combining pathogenetic and etiotropic therapy.

The stages in the treatment regimen include: preparation, prescription of an anthelmintic product against the background of pathogenetic therapy, rehabilitation, medical examination, monitoring the effectiveness of treatment.

Treatment of patients in the acute phase is focused primarily on the relief of allergic reactions and associated organ pathology. For this purpose, antihistamine products (diphenhydramine, suprastin, tavegil), calcium chloride, sedatives (valerian, bromine products) are prescribed. As anti-inflammatory products, taking into account the development of vasculitis, salicylates, butadione, ascorutin are prescribed.

Prevention

Eating only well-cooked and fried, carefully salted fish; protection of water bodies from pollution by faeces.
