Acebutolol (Acebutolol)

Indications for use:
Angina pectoris in case of combination with arterial hypertension (persistent rise in blood pressure); hypertension (persistent rise in blood pressure); arrhythmias (heart rhythm disturbances): atrial tachycardia, paroxysms of supraventricular reciprocal tachycardia, including an atrioventricular node in the chain; tachycardia involving abnormal pathways, flutter and atrial fibrillation (to slow the ventricular rate), ventricular extrasystole.

Pharmachologic effect:
Cardioselective (selectively acting on the heart) beta1-blocker with its own sympathomimetic and membrane stabilizing activity. The negative chronotropic effect (decrease in heart rate) is less pronounced than that of propranolol. Lengthens the time of atrioventricular conduction, increases the refractory period of the atrioventricular connection; at high concentrations, it slows down the conductivity in the His-Purkinje system; depresses the automatism of the sinus node; inhibits conduction along abnormal pathways, increases the refractory periods of the ventricles (but indistinctly the atria). It has a hypotensive (lowering blood pressure) effect, which manifests itself after 2-5 days and maximum after 3-4 weeks. At the same time, it reduces the stroke and minute volume of the heart without increasing the total peripheral resistance (vascular resistance to blood flow). It has an antianginal (anti-ischemic) effect.

Acebutolol (Acebutolol) method of administration and doses:
Intravenous bolus at a dose of 1 mg / kg under the control of blood pressure and heart rate: first, 1/3 of the calculated dose for 5 minutes, then after a 5-minute break and good tolerance, the rest is administered. Inside – 200 mg 2-3 times every day. Can be combined with diuretics (diuretics) and prazosin.

Acebutolol (Acebutolol) contraindications:
Atrioventricular block II-III degree (violation of the impulse through the conduction system of the heart), bradycardia, arterial hypotension, acute and chronic heart failure.

Acebutolol (Acebutolol) side effects:
At the beginning of therapy, fatigue, dizziness, depression (a state of depression), a slight headache, increased sweating, sleep disturbances, nausea, constipation, diarrhea (diarrhea), redness of the skin, itching, a feeling of cold and paresthesia (numbness) in the extremities, a decrease in reaction capabilities. In some cases, bradycardia (slow pulse), hypotension (low blood pressure), atrioventricular conduction disturbances (conduction of excitation through the conduction system of the heart), the appearance of symptoms of heart failure, a decrease in the secretion of tear fluid, conjunctivitis (inflammation of the outer membrane of the eye), dryness in the mouth, reduced potency, hypoglycemic conditions (reducing blood sugar levels) in patients with diabetes mellitus. In predisposed patients, symptoms of bronchial obstruction (narrowing of the lumen of the bronchi) may appear. In patients with bronchospastic symptoms, the simultaneous appointment of beta-adrenergic stimulating products is recommended.
